Kathy Alexis QUALITY IMPROVEMENT MANAGER, CLINICAL PROGRAMS 212-710-3817 kalexis@chcanys.org After her 11-year tenure at the NYC Department of Health and Mental Hygiene, Kathy Alexis joined CHCANYS in March 2008. Kathy brings a wealth of knowledge and experience from her positions as a case manager, senior health educator, and diabetes clinical liaison. She provided patient health education on various health topics such as TB, lead poisoning, asthma and diabetes. She also worked with clinicians on clinical systems improvement efforts in underserved communities across New York City. She holds a Master’s of Public Health (MPH) degree in Community Health Education from Hunter College and is certified as a Certified Health Educator Specialist (CHES). Since 2008, Kathy has been the main lead on the NYS Diabetes Campaign with the New York State Health Foundation working with member organizations statewide to improve their work surrounding diabetes care and management.

Margaret Brennan joined CHCANYS as its first Chief Operating Officer in September 2005. Prior to that, she held executive leadership positions in national and local non-profit health organizations in New York City for 10 years. She has an MPH from the University of North Carolina-Chapel Hill, a certificate in not-for-profit management from Columbia University, and a BA from the University of Wisconsin-Madison.

Mary Lou Caprara joined CHCANYS in February of 2009 and works in coordination with the NYS Department of Health to implement the Healthcare Emergency Preparedness State Contract assisting health centers in the preparation, planning, and coordination required to continue operating during an emergency situation. Mary Lou holds a Bachelor of Arts degree in Liberal Studies/English from The Sage Colleges.

David Davis joins CHCANYS as a program coordinator of the Albany AmeriCorps program. He is responsible for recruiting, training and supervising AmeriCorps members located in Albany and the surrounding areas. David has a Bachelors degree in Psychology and a Masters degree in Social Work from the University at Albany’s School of Social Welfare. He has over 8 years of experience in the Social Welfare field and non profit sector. He has worked with a variety of populations including forensics, high-risk youths, mental health, developmental disabilities, and substance abuse. Alongside his work in the field David has also committed his time and energy to the community and has formulated, organized, and implemented various community outreach programs in the Albany area most notably the Albany Association for Black Social Workers student chapter.

Rebecca joined CHCANYS in August of 2008. She brings over 20 years of broad experience providing operational support to health centers in New York State. Her recent focus has been communicating and directing the implementation of clinical policy to clinic staff and site management in health centers in 5 upstate New York counties. Rebecca has worked very closely with clinical teams to incorporate the Quality Improvement Program into health center operations and education. She holds a Bachelors of Science degree from Russell Sage College.

Mario J. Gonzalez joins CHCANYS as the Emergency Preparedness Coordinator for New York City. He’s been an active Emergency Medical Technician since 1987. He has a Bachelors of Science in Natural Science from St. Peter’s College in Jersey City and a Masters Degree in Emergency Management and Homeland Security from Metropolitan College of New York. Mario is currently a CEM® candidate and a member of the International Association of Emergency Managers. Mario served as the Director of EMS and Emergency Management for the Caritas Healthcare System and opened the first Emergency Management Department in the history of St. John’s Queens Hospital and Mary Immaculate Hospitals. During his tenure at Caritas, Mario designed and implemented the emergency management programs at the Caritas outpatient health centers. As a member of the Disaster Medical Assistance Team NY-4, Mario was deployed as the Logistics Chief to Louisiana as part of the Hurricane Katrina and Rita efforts.

Kate joined CHCANYS in April 2010 as an Administrative Assistant in the Albany office. She has worked for several other not-for-profit organizations, including the SUNY Charter Schools Institute as an assistant to the Director of Public Affairs and the Muscular Dystrophy Association, where she coordinated various projects for its annual fundraising campaign. She holds a Bachelor of Arts degree in English Literature from Hamilton College.

Raquel joined CHCANYS in January 2006 as the Executive Assistant to the COO and the CIO. Raquel has moved into the position of Administrative Coordinator. Raquel will provide administrative support to the Quality Initiatives, Americorps, Finance and Operations and Development staff, continue to take the lead in registration for the Annual Clinical and Statewide Conferences (as well as many other conferences and trainings), and assist in the management of the operations of the New York City office.

Stefanie came to CHCANYS in June 2010 with nearly five years experience in development, fundraising and communications. Immediately prior to joining CHCANYS, she was Development Officer for a growing education-focused nonprofit, where she managed all aspects of the organization’s grant-seeking and reporting activities and was part of a team that guided their successful rebranding effort. She is a licensed attorney and was a litigator for four years prior to entering nonprofit. She holds a Certificate in Nonprofit Management from the Great Cities Institute of the University of Illinois at Chicago, a J.D. from St. John’s University School of Law, and a B.A. in Journalism from New York University.

Lisa has been with the Community Health Care Association of New York State (CHCANYS) since January 2008. In July 2010 she promoted to Vice President, Health IT, responsible for developing and managing a statewide program to support the Health IT needs of New York’s FQHCs. As VP HIT, Lisa will maintain a presence on the state and national level, representing our members' interests in policy discussions and other forums. Prior to this, Lisa served as the first Director of CHCANYS’ Primary Care Health information Consortium, CHCANYS’ Health IT support program for the 30 New York City health centers. PCHIC services include training and technical assistance, fund development, advocacy and user group support. Lisa has over 20 years experience in New York health care, including senior finance and management positions at New York University and NYU Medical Center. She began her career in the finance division of the New York City public hospital system, the Health & Hospitals Corporation. She holds a Master’s in Business Administration from New York University and a Master’s in Public Policy from Harvard University.

Sajjadur joined CHCANYS in June 2007 as a program coordinator. Sajjadur directs and manages the CHCANYS AmeriCorps program. He is responsible for recruiting, training and supervising AmeriCorps members. Sajjadur has previously worked in non-profit development and corporate finance. Sajjadur holds a B.A. & M.A. in psychology from Wesleyan University in Connecticut. Sajjadur concentrated on community psychology and student assessment methodologies for his Masters program.

Dr. Kameron Wells joined CHCANYS in August 2010 as their leader for state-wide Clinical Quality Initiatives (CQI) and brings over 24 years of healthcare experience with him. After working as a critical care nurse for seven years, he returned to school and received his Doctorate in Naturopathic Medicine in 1998 from Bastyr University in Seattle, Washington where he also completed a one year residency at the Bastyr Natural Health Clinic. When he returned to the east coast in 1999, he started his private practice. One year later, he was asked to join the late Dr. Robert Atkins at The Atkins Center for Complementary Care in Manhattan. There, he treated patients with acute and chronic conditions using herbal medicine, homeopathy, therapeutic nutrition and counseling. For the past seven years, Dr. Wells has been working with Community Healthcare Network as their AVP of Ambulatory Care Compliance/QI/Medical Informatics. He worked with senior leaders and managerial staff to acquire and maintain their Joint Commission accreditation status. He worked closely with the CMO and IT Dept. to coordinate and implement their transition to EMR, and then supervised and evaluated that system (eClinicalWorks) from a clinical standpoint. He coordinated the organizational, departmental and programmatic Quality Improvement activities, as well as the agency’s annual QI evaluation. And lastly, he developed and implemented the quality management of their Medical Informatics programs, products and services that not only addressed the clinical needs of the patients, but also provided support and on-going training to their clinical staff.

Sandy joined CHCANYS in March of 2008 following two years of sales and account management within the CHC software industry, and 12 years of financial and operational leadership within a NY health center. She serves as the Director of Information Technology and is involved in various HIT initiatives both state-wide and national. She strives to stay abreast of current and proposed IT regulations and funding opportunities that may affect our health centers. She works directly with New York Health Choice Network as the Project Manager coordinating network activities in relation to eClinicalWorks product implementations and data warehouse design and implementation.

Matthew joined CHCANYS in August of 2008 as Emergency Preparedness Program Manager. Matthew has experience in non-profit disaster recovery and worked to create a coordinated assistance network for 9/11 Recovery Workers in need of case management. He has also played a role in disaster response, coordinating volunteers from multiple agencies to provide services to NYC populations following the August 8, 2007 tornado and flooding in Brooklyn and Queens. He holds a BA in Business Administration from New Mexico State and a Master of Public Administration in Emergency and Disaster Management and Homeland Security from Metropolitan College of NY.
