Bronx Community Health Network: was founded in 1996 to ensure that the socio-economically depressed and medically underserved residents of the Bronx have access to high quality and comprehensive health care services, Bronx Community Health Network (BCHN) is a not-for-profit, community-based organization and Federally Qualified Health Center. The organization provides a wide range of comprehensive, coordinated primary medical, oral, and mental health care and related diagnostic, pharmacy, social support, and enabling services for medically underserved/uninsured residents. BCHN’s service area includes the entire borough of the Bronx, including all 25 zip codes.
BCHN mission: To improve the health of people and families in the Bronx; provide access to affordable, quality health care, especially for uninsured persons; promote disease prevention, early treatment and healthful lifestyles; and obtain financial and other resources for programs and services.
BCHN is seeking a full-time Licensed Practical Nurses who will be a key member of the health care team to provide support to clinicians in addressing the health care needs of patients. The ideal candidate will be responsible for health counseling and the provision of care supportive to life and well-being.
About the Role: The Licensed Practical Nurse is responsible for providing care to patients within the center as directed by the medical staff.
- Coordinates with health care team to facilitate the highest quality of health services for patients.
- Administers medication and observe patient response, as evidenced by documentation in the medical record.
- Supervises referrals and follow-up on patient medical care, including correspondence.
- Assists providers with procedures.
- Confers with physicians/providers when a prescribed medical regimen needs interpretation and/or clarification.
- Provides health counseling to patients, including HIV counseling.
- Follows up on abnormal labs as necessary.
- Maintains nursing competence through participation in continuing professional education and other appropriate learning experience.
- Works assigned shifts at locations other than primary assigned work site, when necessary, as deemed by management and in accordance with BCHN Policy.
- Performs other duties, as assigned.
Qualifications and Skills
- Experience 1-4 years as an LPN
- High School Diploma or GED (Required).
- LPN License (Current New York State License)
- BLS Certification
- Bilingual English & Spanish (Preferred)
Knowledge, Skills, and Abilities Required:
- Proficient in Microsoft Office products.
- Effective project management skills.
- Self-motivated and proactive.
- Ability to work independently and collaboratively in a fast-paced environment.
- Ability to communicate and/or related comfortably and professionally to colleagues and external partners of BCHN.
- Flexible and adaptable regarding learning and understanding new technologies and processes.
- Experience with credentialing software preferred.
- Strong interpersonal skills.
Salary: Exact compensation based on skills; experience $65,000 - $70,000 annually
