Organization Overview:

The mission of the Community Health Care Association of New York State (CHCANYS) is to champion community-centered primary care in New York State through leadership, advocacy, and support of Community Health Centers. To do this, CHCANYS serves as the voice of community health centers, working to ensure that every New York State community has primary care that encompasses all aspects of each patient’s health and well-being. CHCANYS is a dynamic, team-oriented, 501(c)(3) non-profit organization, and as New York State’s Primary Care Association (PCA), advocates on behalf of and provides technical assistance to community health centers (CHCs, health centers) throughout New York State. New York’s nearly 800 CHCs sites provide comprehensive, community-based primary and preventative care services to 2.2 million New Yorkers annually, regardless of insurance status or ability to pay. CHCANYS is nationally recognized for its programs in health policy, performance improvement, health information technology, health system planning, and training and technical assistance (T/TA).

Position Summary:

The Navigator Program Manager will manage the Health Insurance Navigator program. The Program is a community assistance program designed to meet the needs of New Yorkers by providing assistance in convenient, community-based locations. The Navigator Program Manager supervises Navigators who are experts in making it easier for individuals, families, and small businesses and their employees, through the NY State of Health marketplace, to apply for health insurance coverage, understand their coverage options, and enroll in a plan that is right for them. Working in multi-cultural and multi-ethnic work environments, the Navigator Program Manager oversees in-person enrollment assistance for affordable health coverage such as: Qualified Health Plans (QHPs), Essential Plan, Advance Premium Tax Credits, Cost Sharing Reductions, Medicaid and Child Health Plus, and Small Business Marketplace. The Navigator Program provides culturally competent, linguistically appropriate, and disability accessible enrollment services. They are available at convenient times, including evenings and weekends, at no cost to enrollees.

This position will also support CHCANYS in its unique and essential role, along with its member FQHCs, in addressing health disparities and our DEI efforts and policies across CHCANYS. This is a grant-funded position

Responsibilities:

Supervise team of six Navigators - four located in Wayne, Ontario and Yates counties and two located in Erie County.

Oversee training of Navigators and ensure compliance with DOH certification and recertification requirements.

Provide training and technical assistance to Navigators, monitoring productivity and ensuring timely and quality facilitation of application assistance. Track DOH dashboard reports to ensure follow-up and requests are completed by assigned Navigators.

Develop and oversee site schedules of Navigators to ensure that there is adequate coverage throughout the service area, including locations, days, and times. Support Navigators with site relationship management and with the development of community outreach plans. Coordinate Navigator participation at NYS and DOH-sponsored events in Wayne, Ontario, Yates, and Erie counties.

Conduct community outreach to potential enrollees, including educating consumers regarding their benefits and responsibilities under the New York State of Health. Ensure that only State-approved outreach materials are used when conducting community outreach activities.

Maintain thorough records of services provided by the Navigators, including outreach and education activities. Review individual Navigator monthly expense and program reports for accuracy with program director and timely submit monthly program progress report to DOH.

Schedule bi-weekly supervision meetings with Navigators to provide feedback and address issues related to the work. Conduct virtual and on-site site visits as needed. Conduct annual performance evaluations for Navigators.

Provide direct Navigator services to consumers and community members.

Keep current regarding policies, regulations, and rules related to the Navigator Program and health insurance issues, and ensure all Navigators are current on all requirements associated with the program. Participate in monthly DOH-sponsored calls and conduct training sessions or meetings, as necessary.

Coordinate with Service Programs Coordinator on reporting program information for Salesforce and CHCANYS Strategic Plan reports.

Monitor and support sub-grantee Agency with the implementation of the Navigator program. Oversee training and provide technical assistance to two Navigator FTEs staffed by sub-grantee Agency, ensuring compliance with DOH Navigator program requirements, addressing cases, and monitoring performance.

Assist CHCANYS Finance team by reviewing program vouchers for accuracy.

Sign annually and abide by the Certificate Regarding State and Federal Confidentiality Requirements.

Perform other responsibilities as assigned by the Senior Director of Programs.

Qualification Requirements:

  • High School Diploma or equivalent. BA/BS degree preferred
  • Minimum of five years of experience providing assistance to consumers and/or advocates on health insurance and health care access issues, of which two years must have been in a leadership or supervisory role.

· Proficiency with word processing, spreadsheet, and database applications.

· Must be able to travel throughout Yates, Wayne, Ontario, Erie counties, and NYC as required.

· Weekend and evening hours may be required.

  • Ability to work a flexible schedule, including some evenings or weekends
