Awarded Social Care Networks

The Hochul administration has named nine organizations as Social Care Networks (SCNs) and awarded them a combined $500 million in funding over the next three years. These SCNs will identify enrollees and connect them to health-related social needs services to improve health outcomes. The following organizations will cover nine regions throughout the state:

Background on the 1115 Waiver and Social Care Networks 

The 1115 New York Health Equity Reform Waiver, approved in January 2024, spans 3.5 years and allocates $7.5 billion in funding, with approximately $6 billion federally funded. This waiver aims to address health disparities in low-income communities by expanding access to primary and behavioral health services, addressing health-related social needs (HRSNs), and promoting health equity throughout the state. 

A significant portion of the waiver is dedicated to providing HRSN services, such as nutrition services, housing supports, and transportation, to improve overall health outcomes for Medicaid enrollees. Social Care Networks (SCNs) are essential in this effort, responsible for screening Medicaid patients for HRSNs and navigating them to appropriate social care services in collaboration with community-based organizations (CBOs). 

Social Care Network Program and Billing Manual 

The state will soon release the Social Care Network Program and Billing Manual. This manual will clarify key areas for health centers, including standards for social care networks, guidelines on where services can be provided and who can provide them, and information on reimbursement for screenings and services. 

CHCANYS will announce the release and provide an overview, focusing on essential information and how health centers can engage to offer and connect these services for their patients.
