CHCANYS is excited to announce a joint session with Azara providing CHCs with holistic guidance on optimizing health related social needs data within the CPCI for cardiovascular health management, leveraging their custom User Guides. This session will also feature a use case scenario and insights from Community Health Center of Buffalo, and the session will address the following objectives: 

1.  Learn how to apply tools and key functionalities from the CPCI to support hypertension care.

2.  Leverage a team-based approach through practical use case scenarios and workflow steps.

3. Recognize the importance of accurate data collection and validation in hypertension care.

4. Stratify hypertension data by health related social needs and other factors to reduce poor health outcomes.

5. Track and improve hypertension care using cohort monitoring.
