CHCANYS is excited to announce a joint session with Azara providing CHCs with holistic guidance on optimizing SDOH data within the CPCI for cardiovascular health management, leveraging their custom User Guides. This session will also feature a use case scenario with a peer health center, and will address the following objectives:  


  • Learn how to apply tools from the User Guides to support hypertension care. 


  • Leverage a team-based approach through practical use case scenarios and workflow steps. 


  • Recognize the importance of accurate data collection and validation in hypertension care. 


  • Stratify hypertension data by SDOH, race, and ethnicity to reduce disparities. 


  • Track and improve hypertension care using cohort monitoring.
