We’re excited to invite you to a two-part webinar series, Navigating the Heart of Health: A CPCI Review for Cardiovascular Health Promotion in collaboration with Azara Healthcare, aimed at optimizing hypertension care delivery and improving patient health outcomes. 

Part One: Tuesday, May 13th, 2025 | 12:00 – 1:30 PM  

This session is designed for providers, QI leads, and care team members at any level of experience with the CPCI. You’ll learn best practices for using the CPCI in hypertension management, including:  

  • Key CPCI tools that support hypertension care  

  • A user case scenario and workflow map  

  • Data validation in hypertension care  

  • Team-based care strategies and insights from health-related social needs data
