Join CHCANYS on May 28, 2025 at 12:00pm for an expert-led learning session introducing two comprehensive tools designed to help community health centers implement and optimize social risk screening and referral-making practices. These guides focus on addressing health-related social needs (HRSN), such as housing, food insecurity, and access to transportation—key factors that significantly impact patients' health outcomes.
