Luro Health supports FQHCs and CHCs in launching white-labeled weight management programs built from best in class centers at NYU and Mt. Sinai. Patients maintain their physician connection and Luro Health provides a treatment plan that includes nutrition, exercise physiology, mental health, and health coaching. Revenue is shared with the FQHC or CHC.
No risk or cost to the FQHC or CHC partner. Luro Health will set up the entity that delivers care and manages all the staff and billing. For each billable event, Luro Health will work with the partner to determine a profit split and share revenue back to the FQHC or CHC.
The partnership requires the support of the medical director and the IT director to launch the partnership. Determine the referral pathways and setting up the agreement for coordinating care.
