CHCANYS is currently planning to recruit for a second cohort of our Care Team of the Future Collaborative in partnership with Coleman Associates. This cohort will launch in early 2023.
The goal of this collaborative is to decrease no show rates, improve access to appointments for both new and established patients, establish systems to enhance continuity of care, decrease cycle time for visits, develop and strengthen your model for team-based care, establish a better process to support provider and staff work-life balance, build a model of care that provides a framework for a ready transition to Value–Based Reimbursement, create a platform for collection/discussion/interpretation of key operational data, and create the necessary foundation for true transformation based out of the concepts of a Patient Centered Medical Home.
