CMS has announced the launch of the Making Care Primary (MCP) Model on July 1st with 133 participants. This ten-year model aims to improve the delivery of advanced primary care services for Medicare enrollees. MCP features three progressive tracks: building infrastructure, implementing advanced primary care, and optimizing care and partnerships. The model will be tested in eight states with 133 participants representing 772 practices, including 55 participants that are Federally Qualified Health Centers and 94 participants that are small primary care organizations with 5 or fewer practices. New York State is one of the eight states and only upstate health centers were eligible to apply. Four health centers from New York joined this model.
