HRSA has released the next round of funding for the Health Center Controlled Networks (HCCNs). HCCNs support health centers to better leverage health IT and data to improve the delivery of affordable, accessible, and high-quality primary health care. CHCANYS is in the process of preparing our application and would like your continued support of this project by participating in the NYS-HCCN.
CHCANYS strongly encourages all members to participate in the HCCN program – being a part of the network is free, and allows you to receive timely, relevant, high-value health IT-related programming, CPCI financial subsidies, and a wide variety of training and technical assistance opportunities that respond to centers’ needs and the changing NYS landscape. Participating in the HCCN community will also provide opportunities for your health center to inform and shape CHCANYS’ health IT programming. The focus for the 2025-2028 HCCN program activities include these key areas:
- Data management and analytics and where applicable, AI, to enhance clinical, financial, and operational decision-making,
- Interoperability of systems and digital health tools to streamline the ability to exchange healthcare data and reduce duplication of services,
- UDS+ implementation to optimize UDS data reporting and performance measurement,
- Value-based care (VBC) to increase the use of data to update and deliver value-based care, enhancing patient and provider experiences, improving health outcomes, and reducing health disparities, particularly for the uninsured,
- Social Risk Factors to increase the number of health centers implementing integrated and standardized processes and procedures for screening, referrals, and care management for health-related social needs.
A signed MOA is a requirement for all centers to join the network for the 2025-2028 project period. Health center program participation is voluntary, requirements are minimal and help to secure continued HRSA funding for CHCANYS’ HCCN.
