The Center for Medicare and Medicaid Innovation (CMS Innovation Center) is accepting applications for the forthcoming Making Care Primary (MCP) Model. Please note that health centers participating in ACOReach, or those located in Westchester, NYC, and Long Island are ineligible to participate. Interested organizations are encouraged by CMS to begin their applications even if they are not prepared to submit at this time. Eligibility requirements and additional model details can be found in the Request for Applications (RFA). Interested organizations must submit their application via web portal by Thursday, November 30 at 11:59 PM EST to be considered for the model.
