The NYS Cardiovascular Health Initiative is a collaboration between CHCANYS and NYSDOH that acknowledges the interconnectedness between cardiovascular health, social needs, and overall well-being. We empower health centers to adopt a holistic approach to care, prioritizing effective screening, team-based care, and strengthened community connections.
The guides leverage insights from clinical, behavioral health, race and ethnicity, and social drivers of health (SDOH) data within Azara’s CPCI/DRVS. They provide a data-driven approach to improving equitable cardiovascular health outcomes by integrating key tools, reports, and functionalities into your workflows.
