NYS Social Care Networks (SCN) Program Overview provides a general outline of New York State’s Health Equity Reform 1115 Waiver Amendment and the 9 regional Social Care Networks (SCN) that were established to deliver health-related social needs (HRSN) services to Medicaid members. SCNs are tasked with developing a strong network of Community-based Organizations (CBOs) and other entities that provide HRSN screenings and care coordination.
