A new draft of the SCN Operations Manual has been released with an important clarification for health centers. Previously, it was unclear if Social Care Network (SCN) services—screening, navigation, and enhanced HRSN services—could be reimbursed on the same day as a threshold visit.
The updated manual now confirms that SCN services can be reimbursed separately on the same day as threshold visits.
Health centers can now provide SCN services while patients are already on-site (or remote) for other visits, ensuring better access to care and more streamlined workflows.
In addition to this update, DOH has indicated they will release a list of grants and programs with clear guidance on how these will interact with SCNs and whether they represent duplicate funding.
