JOB TITLE:               Nurse Care Coordinator

The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.

POSITION SUMMARY:

The Nurse Care Coordinator will join the Care Management division with the goal of assisting targeting high acuity patients who are “high-cost-high needs” patients: those with multiple or complex conditions, often combined with behavioral health problems or socioeconomic challenges. The Nurse Care Coordinator would develop partnerships and trust with patients, facilitate communication between patients and care providers, and empower patients with knowledge of disease care.

RESPONSIBILITIES:

  • Review and reach out to high cost, high utilizer patients.
  • Follow-up on hospitalization/transitions of care.
  • Focus on Gaps in Care.
  • Participate in the development/documentation /review and update of client centered comprehensive, integrated, interdisciplinary care plan in consultation with Care Manager and other team members to ensure focus on desired outcomes.
  • Conduct Home visits as needed.
  • Maintain effective communications with clients, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources and other Agency staff on behalf of clients.
  • Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan.
  • Create protocols and standard work flows
  • Frequency, modes, and duration of patient contact-Monthly telephonic or in-person visits.
  • Document in electronic health record.
  • Discharge patients from clinical care management to lower level of care, as needed.
  • May be assigned other tasks and duties reasonably related to the job responsibilities.

QUALIFICATIONS:

  •  Between 0-2 years of relevant experience in healthcare, experience with care management preferred.
  • Licensed as a Registered Professional Nurse in the State of New York.
  •  Basic CPR (BCLS) Certification required along with Infection Control Certification.
  • Working knowledge of computer software and electronic health record systems.
  • Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
  • Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental                  treatment model.
  • Knowledge of Medicaid, Social Security and other entitlements preferred.
  • Excellent interpersonal skills required.

SALARY:                               Commensurate with experience
