CHCANYS Public Health AmeriCorps is a partnership and collaboration between CHCANYS, the New York State Commission on National and Community Service, AmeriCorps, and the CDC.

Presented by NYC Health + Hospitals: EpicCare Link is a web-based platform that provides real-time access to patient information so that community providers can access their patients’ clinical data and refer their patients to our sites for services.
