Position Summary:
The BH Integrated Services Coordinator is a management position with direct patient care responsibilities. The role involves strengthening DEI initiatives, managing patient care processes, onboarding new staff, and providing supervision to behavioral health providers. The coordinator plays a key role in reviewing workflows, facilitating smooth transitions for medical patients seeking behavioral health services, and coordinating chart reviews across clinics. On-call duties, employee evaluations, and addressing personnel/HR issues are part of the position. The coordinator works at multiple sites, remotely, and actively participates in operations meetings, contributing to the integration of behavioral health services in primary care clinics.
Some Key Responsibilities:

Collaborate with Callen-Lorde's management team, staff, and DEI committee to strengthen Diversity, Equity, and Inclusion efforts, fostering an anti-racist and anti-oppressive culture and work environment.

Collaborate with managers as the on-site administrative contact for OMH behavioral health teams, evaluating and enhancing workflows at OMH-licensed sites to optimize patient care.

Manage the behavioral health assessment and referral process for patients from various departments at Callen-Lorde, ensuring engagement in care with outside agencies and assigning teams and modalities for patients with acute health and behavioral health issues referred from primary care or inpatient units.

Manage the waitlist for services, facilitating urgent scheduling with appropriate therapy and psychiatry assignments based on patient need, and manage patients request for re-assignment of psychotherapist

Facilitate structured admissions, transfers, and discharges at OMH-licensed sites by reviewing documentation, making recommendations, and ensuring appropriate referrals for ongoing care.

Assist in the development and maintenance of effective and appropriate mechanisms for interdepartmental referrals, case conferencing, and communication.

Work to ensure provider performance benchmarks are met, including the provision of high-quality behavioral health care, documentation aligning with OMH, DOH, DOHMH, HRSA, and CMS regulations, and operational benchmarks including visit volume and no-show rates.

Provide clinical supervision to Behavioral Health Providers. In tandem with other managers, participate in hiring process, complete provider evaluations, ensure timely completion of encounters, accurate documentation aligning with OMH, DOH, HRSA, and CMS requirements, and ensure visit volume benchmarks are met.
Education, Experience
- Masters Degree in Social Work and NYS licensure as an LCSW (Licensed Clinical Social Worker)
- A minimum of three (3) years of postgraduate experience in a health /behavioral health, and/or social services setting preferred
- Previous supervisory experience preferred
- Knowledge of lesbian, gay, bisexual, transgender, and HIV health/ behavioral health issues and concerns
- Demonstrated ability to work with multiple evidence-based approaches to behavioral health treatment
- Demonstrated ability to coordinate the provision of direct social services, problem-solve, and optimize service delivery systems.
