WHO WE ARE & PURPOSE OF POSITION
For over 50 years, Covenant House New York (CHNY) has been a leader in providing a full continuum of shelter, transitional and permanent housing and wrap-around support services for vulnerable homeless, runaway and exploited youth. As the largest provider of runaway and homeless youth services in New York City, CHNY serves more than 1,500 youth and young adults ages 16-24 annually. Our doors are open to all young people who need help, regardless of their race, religion, sexual orientation, gender identity or expression. We provide shelter, housing, education, employment, health, mental health and other services using harm reduction and trauma-informed principles. We also advocate for systemic change through public policy while promoting awareness of critical issues related to youth homelessness. Working in our Federally Qualified Health Center and under the direction of the Medical Director, and in collaboration with the Health & Wellness Center team, the Registered Nurse (RN) will focus on long-term acute care and preventative treatment of at-risk individuals. The RN will also provide telephone triage, arrange lab work, administer medications, and provide education outreach. This position will be hired full-time within the Health & Wellness Center and work with patients during all operating hours, including weekend hours. This position will also play a pivotal support role in HIV prevention, testing, education, and patient care support. This position requires in-person presence at CHNY’s Manhattan location; there is no opportunity for remote work.
DUTIES AND RESPONSIBILITIES
(Responsibilities to include but not limited to the following)
- Be an integral part of the patient care team and assist with patient care within the scope of Health & Wellness Center
- Ensure clinical planning, coordination, assisting with HIV testing, and patient triage
- Obtain patient medical history, monitor patients’ vital signs, administer vaccines, document patient information in eClinicalWorks
- Support with the administration of medication, treatments, and evaluating patients’ response to medication and treatment
- Provide long term care for patients living with HIV including but not limited to; performing lab testing every few months to check CBC, chemistry panel, viral load, CD4
- to monitor immunity, and ongoing STD screening. Assist in determining the next steps in care, monitor for high-risk behaviors that place the patient more at risk for transmitting HIV/AIDS or other STDs and provide counseling and education to reduce the risk, monitor the patient's response to HIV medication, not only to include lab work but also tolerance to the medication, and monitor for any psychosocial needs the patient may have and ensure appropriate referrals to mental health supports
- Clear medication for shelter residents and document all cleared medication in ETO software
- Monitor and revise the active medication list for youth patients
- Maintain and track medication inventory
- Review patient charts for missing labs, reports, and/or open orders
- Plan, articulate, and coordinate care and interventions
- Coordinate and participate in daily huddles
- Facilitate clinical messages and chronic condition management education
- Ensures that the quality of care and environment are acceptable
- Enforces adequate Infection Control standards, with guidance from the Medical Director
- Work closely with our clinical team members to develop policies and protocols and ensure all policies are adhered to
- Performs other duties as assigned and/or needed
Qualifications
EDUCATION & EXPERIENCE
- Graduated from an accredited School of Nursing. Bachelor’s degree in nursing required
- Current License to practice as a Registered Professional Nurse in NY required
- BLS required upon hire
- Must have 2-3 years’ experience in a Federally Qualified Health Center or Urgent Care with a strong preference for experience working with HIV testing, education, and treatment
- Proficiency in Electronic Health Record, eClinicalWorks preferred
- Strong clinical skills
- Strong technical skills, including phlebotomy, medication administration
- Excellent Communication (both oral and written), interpersonal, and organizational skills
- Excellent organizational and time management skills
- Compassionate and understanding for people experiencing Homelessness
Schedule 9:00 am - 5:00 pm
Annual salary $109, 200.00
