Social Determinants of Health
According to the World Health Organization, Social Determinants of Health are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks. Collection of data related to Social Determinants of Health (SDOH) can positively impact systemic population health improvement as well as engage patients in addressing their non-medical needs by coordination and increasing access to appropriate services.
